As the social worker, you’ll serve as a part of the multidisciplinary care team, advocating for residents and assessing their psychosocial needs to enhance their well-being.

About The Role 

*This position is 24 hours per week, typically worked over three days (Monday - Thursdays)

This role is integral to addressing the psychosocial needs of residence members, promoting their overall well-being, and fostering a supportive and inclusive environment. The position provides both team and program leadership, while also playing a key role in health and safety oversight.

The role models the Wellness Model of Care and supports the team in delivering person-centred services. It involves building strong relationships with residents, their loved ones, team members, and external agencies, while coordinating services and resources to meet individual needs.

Key Responsibilities 

Resident Care & Psychosocial Support

  • Address the psychosocial needs of residence members and promote their overall well-being.
  • Provide adjustment counselling and support residents through transitions and changes in care.
  • Provide crisis intervention and debriefing support to residents, their loved ones, and team members.
  • Lead resident care conferences and facilitate end-of-service planning, when required.
  • Collaborate with the care team to develop and implement personalized service plans.
  • Model the Wellness Model of Care and support the team in applying its principles.

Program & Team Leadership

  • Provide team and program leadership to support effective service delivery.
  • Lead and facilitate resident support groups and other psychosocial programs.
  • Assess eligibility for financial and material assistance and connect residents with appropriate resources.
  • Advocate for residents in accessing legislative programs, benefits, and community resources.
  • Build and maintain effective relationships with residents, families, team members, and external agencies.
  • Develop and revise policies and procedures as required.

Resident Placement & Quality Improvement

  • Coordinate bed matching and support appropriate placement decisions.
  • Maintain and manage the private pay waitlist.
  • Participate in continuous quality improvement initiatives.
  • Identify opportunities to enhance programs, services, and resident outcomes.

Health & Safety

  • Identify and address unsafe situations within the residence.
  • Promote and support safe work practices among team members.
  • Ensure health and safety practices are consistently followed.
  • Contribute to building and maintaining a strong culture of health and safety within the residence.

Requirements 

  •  Minimum bachelor’s degree of Social Work. A Master of Social Work is preferred.
  • Current registration in good standing with the British Columbia College of Social Workers
  • Minimum of one year of experience providing services to seniors, or an equivalent combination of education and experience.

Compensation & Benefits 

A competitive salary ranging from $44.85 to $56.02 by the HSP Master Collective Agreement. Vacation entitlement set at 4 weeks, prorated by the FTE. You'll also benefit from employer-paid benefits and an RRSP matching plan after three months of employment.
